NDIS - Support Coordination
Do you have a NDIS plan that includes Support Coordination? NDIS can be tricky to navigate. Our Support Coordinator has the experience to make your NDIS journey easier for you, your family, and your representatives, explaining what services are available for you to be able to make informed decisions. You will also be supported to decide how you want your services to be provided and who you want to provide them. Kerryn will help you find the right providers that best suit your needs, and describe the services that are being implemented. She can organise Service Agreements according to your allocated budget, help with plan reviews to ensure you receive the funding you need to reach your goals, and help you make the most of your funding by giving you options that best suit your needs and goals.
- Your Initial Consultation can be conducted 2 ways – Face to Face giving you the benefit of meeting your Support Coordinator or virtually- a great tool that works in situations when a Support Coordinator cannot see you in person due to various circumstances. This can either be conducted over the phone or through different online platforms.
- During your initial consultation we will read through your goals together, those stated in your plan, and any other goals you have that are not stated. This is to get a better understanding of what you want to achieve during this plan and how we can best assist you.
- We will discuss what you need/want from a Support Coordinator including things like potential providers, assistive technology needs, support workers, etc. to maintain your choice and control.
- Signing of our Service Agreement allows us access to the Support Coordination funds in your plan. This form includes your and Better Off Support Services’ Rights and Responsibilities whilst receiving services. It also discusses the cancellation policy and how to provide feedback.
- We will ask you to sign a consent form, this form gives you the choice and control over how we utilise, gather, and provide your information. You can withdraw or update your consent form at any time.
What is required at the Initial Consultation
If you have information that relates to your disability it may be useful for your support Coordinator to review. E.g., NDIS Plan, Allied Health reports, medication list, etc. Do not be concerned if you cannot gather all your information, your Support Coordinator can assist you with this.

Budget Management
1. Self-managed
This is where you are able to claim money from your NDIS plan to pay providers. Providers will invoice you directly for the supports that you have agreed for them to provide.
2. Plan-managed
Your plan management company will make claims and pay your providers on you behalf for supports you have agreed they will provide.
3. – NDIA-managed
Providers will claim payment directly from your NDIS plan based on your active service bookings. NDIA managed participants must always use providers that are NDIS registered.
Plan Reviews
Your Support Coordinator will gather all the evidence obtained during the course of your plan from your different stakeholders, along with a Support Coordination plan review report. It will then be uploaded to NDIS for the plan review meeting. BOSS’s Support Coordinator is also there for you to assist during the plan review meeting to make sure you get the funding required for the duration of the next plan

Monday – Friday 8am – 430pm
How does a plan review work
8 weeks prior to the end date of your plan, your Support Coordinator will gather all reports and recommendations from your Allied Health and support workers. This information will then be uploaded to the NDIS portal in time for the plan review date. Your Support Coordinator will also write a report that will be uploaded to the NDIS portal. We will also talk through your goals and update these accordingly.
